This journey can be made more easier ifa proper communication bridge can be made betweenboth of them. This paper describes an alliterative C-basedflow-chart to describe a Collaborative Conversation Mapfor chronic disease care. It crafts a map which serves as achecklist for chronic health care providers, especially thosedealing with obesity and diabetes.
